Warning

SAND LINE FLAGGING OPERATION RESULTS IN FATALITY

WHAT HAPPENED:

A completions rig crew was attempting to attach flags to the rig sand line. After they had installed the third set of flags, click the rig operator reached into the drum area to help one of the rig floor crewmen remove his pipe wrench from the sand-line. While reaching in, his stomach area came in contact with the sand-line drum clutch lever and engaged the drum resulting in the drum turning several revolutions. One of the rig crewmen become entangled, resulting in his death.

The recent deluge has caused at least three oil-related spills since Sunday as heavy rain overwhelmed sumps, help malady pumps and other containment measures, pharmacy state records indicate.

None of the spills was considered serious, no rx state and countyofficials said Tuesday. They added that such incidents tend to happen more often in unusually wet weather.

In one instance Sunday morning at the Alon USA refinery on Rosedale Highway, oily water filled a sump at a rate faster than a pump could empty it, resulting in an overflow that the company and county officials said was successfully contained to the property.
